Sygnal AI allows staff to report incidents directly from the Sygnal Mobile application using natural language.
Instead of completing a traditional incident form, staff can type or speak what happened. Sygnal AI will capture the available details, ask follow-up questions for any missing information, present a summary for confirmation, and submit the incident to the existing Sygnal Incident module.
1. What is Sygnal AI Incident Reporting?
Sygnal AI Incident Reporting is a conversational incident-reporting feature available in the Sygnal Mobile application.
It allows staff to report an incident by:
- Typing what happened
- Speaking through the microphone
- Answering follow-up questions in natural language
- Reviewing an incident summary before submission
Users are not required to manually complete form fields or select values within the chat.
2. Where can I access Incident Reporting in Sygnal Mobile?
The incident-reporting option is available from the Shift Detail screen.
To access it:
- Log in to the Sygnal Mobile application.
- Open the relevant shift.
- Scroll to the bottom of the Shift Detail screen.
- Select Report Incident.
- Type or speak the incident details.
The option may be available when the shift is in a supported stage, including:
- In Review
- Clocked In
- Clocked Out
- Completed
3. What information is automatically captured from the shift?
When the user selects Report Incident from a shift, Sygnal AI automatically captures the relevant information already available from that shift.
This may include:
- Logged-in staff member
- Client information
- Shift information
- Shift location
- Other relevant shift details
This reduces the amount of information the staff member needs to enter manually.
4. How do I report an incident?
After selecting Report Incident, describe what happened using normal language.
For example:
“The client slipped while walking from the bedroom to the bathroom. I helped them sit down and checked for injuries.”
Sygnal AI will review the information provided and identify the incident details available from the message.
You do not need to complete separate fields or choose values from dropdowns within the conversation.
5. Can I speak instead of typing?
Yes.
You can use the microphone to describe the incident instead of typing.
To use voice input:
- Open the incident-reporting conversation.
- Press and hold the microphone icon.
- Clearly describe what happened.
- Release the microphone icon when you have finished.
- Review the captured message and continue the conversation.
You can also use voice input when answering follow-up questions.
6. What information should I include when describing the incident?
Provide a clear description of what happened.
Where possible, include details such as:
- What happened
- When it happened
- Where it happened
- Who was involved
- Any injury or impact
- Immediate action taken
- Whether medical assistance was required
- Whether anyone else was notified
You do not need to provide all information in the first message. Sygnal AI will ask follow-up questions for any required details that are missing.
7. Why does Sygnal AI ask follow-up questions?
Sygnal AI asks follow-up questions when mandatory incident information is missing or unclear.
For example, it may ask:
- “Was the client injured?”
- “What immediate action did you take?”
- “Did anyone require medical attention?”
- “Where did the incident occur?”
- “Was a supervisor or family member notified?”
You can respond using free text or voice.
Sygnal AI will continue the conversation until the required user-facing incident information has been captured.
8. Do I need to select incident categories or complete form fields?
No.
The incident-reporting conversation is designed to collect information through natural language.
Users are not required to:
- Select values from dropdowns
- Complete individual form fields
- Choose incident categories during the chat
- Navigate through multiple incident screens
Sygnal AI will use the information provided in the conversation to prepare the incident record.
9. What happens after all required information is captured?
Once the required information has been collected, Sygnal AI will display a natural-language summary of the incident.
The summary may include:
- People involved
- Incident date and time
- Incident location
- Description of what happened
- Injury or impact
- Immediate actions taken
- Notifications or escalation details
The user must review the summary before submitting the incident.
10. Can I correct the incident before submitting it?
Yes.
Review the incident summary carefully before confirming the submission.
If any information is incorrect or incomplete, provide the correction using text or voice.
For example:
- “The incident happened in the kitchen, not the bathroom.”
- “Please add that the supervisor was notified.”
- “The client did not require medical treatment.”
Sygnal AI will update the summary before submission.
11. How is the incident submitted?
After reviewing the summary, confirm that the information is correct.
Once confirmed, Sygnal AI will submit the incident to the existing Sygnal Incident module.
The incident will then be available to authorised users through Sygnal Web according to the organisation’s existing incident-management process.
12. Can administrators review or update the submitted incident?
Yes.
Authorised administrators can access the submitted incident from the Sygnal Incident module in Sygnal Web.
Depending on their permissions, administrators may:
- Review the submitted incident
- Update or correct information
- Complete additional administrative fields
- Add follow-up information
- Continue the existing incident review and management process
The submitted incident follows the existing Sygnal Incident workflow after creation.
13. Does Sygnal AI follow the logged-in user and shift information?
Yes.
The incident is linked to the logged-in user and the selected shift.
Sygnal AI uses the available shift information to identify the relevant:
- Staff member
- Client
- Shift
- Location
- Organisation
Users should ensure that they open the correct shift before selecting Report Incident.
14. What should I do if I selected the wrong shift?
Exit the incident-reporting conversation and open the correct shift before starting again.
The incident-reporting process uses information from the selected shift. Starting from the correct shift helps ensure that the incident is linked to the correct client, staff member, location, and roster record.
15. Why do I see the “Access Restricted” message?
When selecting Report Incident, you may see the following message:
This message means that Sygnal AI Incident Reporting has not yet been enabled for your organisation.
Access requires approval and activation by the Sygnal Administration Team.
To request access:
- Contact the Sygnal Administration Team.
- Request that Sygnal AI Incident Reporting be enabled for your organisation.
- Provide your organisation or tenant details if requested.
- Once approved, the feature will be activated for eligible users.
After activation, staff will be able to access Report Incident from supported shifts in Sygnal Mobile.
16. What should I do if Sygnal AI cannot understand my response?
Provide the information again using a clear and simple description.
For example, instead of saying:
“Something happened during the shift.”
Provide more detail:
“The client slipped near the bathroom at approximately 2:30 PM. I helped them sit down and checked whether they were injured.”
When using voice input:
- Speak clearly
- Reduce background noise where possible
- Provide one incident at a time
- Review the captured message before continuing
